No-Shows & Late Cancellations
Due to an increase in "No-shows" and late cancellations, we will be charging a $35.00 fee for any person not arriving by their scheduled appointment time.
No Grace Period: This office does not have a "grace period". If you have not checked in by your appointment time you will need to pay a $35.00 fee, be re-scheduled to the next available appointment, and will not receive a work note or medications until your next appointment date.
Cancel / Rescheduling
Please contact our office at 808-935-6353 at least 24 hours prior to your appointment if you are unable to make it. If leaving a voicemail after hours, our system will time stamp the date/time of your message.
Financial Responsibility
Failure to cancel 24 hours prior to a scheduled appointment or having a Missed Appointment will result in a $35.00 fee which will be the responsibility of the patient and not the insurance company. You will not be rescheduled until this fee is paid in full.
