How HBOT Works

Under normal conditions, oxygen is carried through the body by hemoglobin in red blood cells. This delivery system has limits: it can only deliver as much oxygen as hemoglobin can carry, and it can only reach tissue that has adequate blood flow. Injured tissue, particularly tissue with damaged capillaries, edema, or compromised circulation, receives less oxygen than it needs to heal. The healing process stalls because the cellular machinery that produces collagen, fights infection, and builds new blood vessels requires oxygen as fuel.

HBOT bypasses this limitation. By breathing 100% oxygen at pressures 1.5 to 3 times normal atmospheric pressure, oxygen dissolves directly into the blood plasma, cerebrospinal fluid, and interstitial fluid at concentrations 10-15 times higher than normal. This dissolved oxygen reaches tissue that red blood cells cannot, penetrating areas of swelling, damaged capillaries, and compromised circulation to deliver the oxygen that the healing process demands.

HBOT as an Adjunct to VMG's Treatment Protocol

VMG does not use HBOT as a standalone treatment for work injuries. It is an adjunct that enhances the healing response initiated by PRP therapy, injection therapy, and other interventional treatments. The sequencing matters.

Post-PRP Recovery

After a PRP injection, the concentrated growth factors need oxygen to drive the collagen synthesis cascade. HBOT sessions in the days and weeks following PRP deliver that oxygen directly to the treatment site at concentrations the circulatory system alone cannot achieve. The clinical rationale: PRP provides the growth factor signal, HBOT provides the oxygen substrate. Together, they produce a stronger and faster tissue repair response than either treatment alone.

This combination is particularly relevant for chronic tendon conditions that have poor blood supply: lateral epicondylitis at the common extensor origin, plantar fasciitis at the calcaneal insertion, and rotator cuff tendinopathy at the supraspinatus insertion. These areas are naturally hypovascular (low blood flow), which is part of why they degenerate in the first place. HBOT addresses the oxygen deficit that makes these tendons slow to heal.

Post-Injection Recovery

After epidural steroid injections, joint injections, or nerve blocks, HBOT can reduce post-procedure edema and accelerate the anti-inflammatory response. This is particularly useful for patients who need to return to work on modified duty as quickly as possible after an interventional procedure.

Chronic Injury Support

For injuries that have been undertreated or mismanaged for months, HBOT helps break the cycle of chronic inflammation and impaired healing. Patients who come to VMG after stalled treatment with a previous provider often have tissue that has been stuck in a low-grade inflammatory state with inadequate oxygen delivery. HBOT resets the oxygen environment, creating conditions where the tissue can respond to the interventional treatment that follows.

★ HBOT Is Not a Wellness Treatment at VMG

Some practices market HBOT as a general wellness, anti-aging, or performance optimization service. VMG uses HBOT as a clinical recovery tool prescribed by Dr. Vally as part of a treatment plan for specific injuries and conditions. The sessions, frequency, and duration are determined by the diagnosis and the treatment protocol, not by patient preference. What a Session Looks Like

HBOT sessions at VMG are straightforward. You enter a pressurized chamber, breathe normally (the chamber delivers 100% oxygen at increased pressure), and rest for 60-90 minutes. You can read, listen to music, or sleep during the session. There are no needles, no medication, and no anesthesia.

During pressurization, you may feel fullness in your ears similar to descending in an airplane. You equalize by swallowing or yawning. Once at treatment pressure, the sensation resolves and you breathe normally for the remainder of the session. Depressurization at the end follows the same gradual process.

There is no recovery time after a session. You can drive, return to work, or continue your daily activities immediately. Some patients report mild fatigue after the first few sessions, which typically resolves as the body adjusts to the protocol.

Contraindications: HBOT is not appropriate for all patients. Untreated pneumothorax is an absolute contraindication. Relative contraindications include certain types of ear and sinus conditions, some seizure disorders, and specific medications. Dr. Vally evaluates every patient for HBOT candidacy before prescribing sessions. This evaluation is part of the overall treatment planning process, not a separate appointment.
